When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.
We are committed to providing dental care that is both high quality and affordable. The fee for your endodontic treatment will be calculated on the extent and complexity of treatment. During your initial consultation we will perform a thorough examination, discuss the probable number of visits, their duration, and the fees involved. For your convenience we accept Visa, Mastercard, American Express, Discover, Care Credit and personal checks. We are pleased to offer Care Credit to our patients. Care Credit offers convenient, no initial payment, interest free payment plans for dental treatment. Offering Care Credit allows us to make the necessary endodontic treatment affordable. We are in network with most insurance companies, PPO’s and DHMO’s. We also file the insurance for you and accept assignment of benefits. Patients are asked to pay an estimated portion of the fee not covered by dental insurance at the time of service. In order to give you the most accurate estimate, please provide us with your dental insurance information prior to the appointment. Any remaining balance in your account after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated.
